So that I can find the best placement for my puppies, I ask that you be honest and complete the form below and return it to me.  Use another sheet or copy and paste the info into your e-mail.  We can also discuss these questions by phone if you prefer, 425-348-5510 or e-mail your application to: Recognize that  I am a small  home breeder with only  two females that I breed once a year for one Spring and one Fall litter.  If you would rather not  wait for your puppy, I'd suggest you contact other breeders in your area. Deposits are only accepted when you actually choose your forever puppy.
